Choosing a family physician and switching family physicians

Patients have the right to choose their family physician (family doctor) and to switch family physicians.

Registration to a family physician’s patient list is based on a written application that the patient must file with the family physician.

Newborns are automatically registered with the patient list of their mother’s family physician.

Each family physician has their own service area, mostly a municipality or municipalities (e.g. city of Tartu, Setomaa rural municipality, Jõhvi rural municipality etc.).

Choosing a family physician and switching family physicians, it is important that

  • the address of the  patient’s residence registered in the population register falls within the family physician’s service area;
  • less than 1,600 persons are registered in the family physician’s patient list (goodwill based cooperation agreement „Ensuring sustainability of primary medical care“ contracted on March 19, 2019).
     

A family physician registers a person on their patient list if

  • the address of the  patient’s residence registered in the population register, noted on the application, falls within the family physician’s service area; or
  • a family member of the patient is already registered in the family physician’s patient list.
     

A family physician may refuse to registers a person on their patient list if

  • the address of the  patient’s residence registered in the population register, noted on the application, does not fall within the family physician’s service area; or
  • the number of persons on family physician’s patient list exceeds the maximum assigned to the family physician (2,000 persons or 2,400 persons when family physician works together with an assistant doctor).

How to register to the family physician’s patient list?

  • Submit your completed application to your chosen family physician.
  • Application to be registered on a family physician’s patient list
    Download application form in English (RTF)  and in Russian (RTF)  language.

You may file the application to the family physician electronically (as digitally signed) or by regular mail.

A new application must be completed and filed for each family member.
Registration on the family physician’s patient list is done only based on relevant application signed by the patient (for minors based on the application signed by a parent or guardian).

 

  • The family physician notifies the person that filed the application of their registration or refusal to register them on the family physician’s patient list.

The family physician notifies the person that filed the application in writing within seven working days as from filing of the application. If registration is refused, the family physician also indicates the reason therefor.

New patients are registered to the family physician’s patient list on the first day of the next calendar month; however, family physician care is provided starting from the day the application for registration to the patient list was filed.
